How to Use a Contact Web Form to Capture Volunteer Information

1. To see how to create a Contact Web Form, please see the document that’s titled ‘Contact Web Form’ in the Eleo Library.

2. When you get to Step 4 (Fields on the Web Form Wizard), you will see this screen. At the bottom you will see a few Volunteer fields that you can add to the form and make required if you would like. If needed, you will have an opportunity to add additional fields later.

3. Once the Contact Web Form is created, you may click on the ‘Advanced’ button on the Web Form Dashboard to make additional modifications.

4. Next, click on the ‘Fields’ tab to add more fields.

5. For example, if you want to add Volunteer Skill Types, click on ‘Add Field’.

6. You will need to know what type of field you want to add. In this case, Volunteer Skill Types are checkboxes on the contact record, so we will select this. Press ‘Continue’.

7. You can change the name of the Field Label to whatever language you would prefer. This will appear on the form, so it should identify the field. We can call this one ‘Skill Types’. Then, for the Field Mapping, choose the field where you would like this information to populate in Eleo. You may leave this required or remove that option. Also, you may write a description (if needed), and choose to display that on the form, or as help. Once you are done, press ‘Continue’.

8. Once it loads, you will see a preview of your new field. If it looks great, press ‘Save’, otherwise press ‘Back’ and make any modifications.

9. As needed, repeat these steps to add as many additional fields that you would like for this form.

Once someone fills out the form, it will look something like this under the ‘Pending’ tab if you view data.


Then, if you exit that view, you may click on ‘Add’. Next, click on ‘Process’.

The record will either update the existing record it matched with or create a new record.


If you search for that record, you will see all of the volunteer data that they completed since the Web Form is now updated in the Eleo contact record.
